Here's how it works

Scan the custom QR-Code or log into your company's custom webpage on your mobile device for contactless check-in.

A custom QR-Code will be created for your company which will direct your staff or guest to your custom webpage. A URL will also be provided in case your staff or guests have trouble using the QR-Code.

Fill out the online form and answer a few simple questions

This form will include the basic input fields needed to comply with the CDC Guidelines. However, you may add additional fields to customize your form to better fit your organization's needs.

Submit the form and recieve your confirmation email of compliance

Your staff or guest will receive a confirmation email. Their information will be stored and will be emailed to your administror(s) daily with a log of all the entries for that day.

If a staff member or a guest answers "YES" to any of the questions or logs a temperature of over 100.3° they could possibly be endangering others. If this is the case, an email will be sent to your administror(s) immediately after they submit the form.
